Hook it around your shoulder. Go from under your ass, over the shoulder and to your penis head. Should work fine.

I think it depends more on your starting girth. If you are 4.5in girth and you buy the penomet, your balls are gonna get sucked into that thing like a million dicks in my ex-wifes mouth. You gotta get one that fits your starting size.
